Continuing Care and Rehabilitation Program

 
The Continuing Care and Rehabilitation Program is a multi-faceted program focused on attaining the highest quality of life for individuals by maximizing abilities, maintaining or improving independence and assisting persons to compensate for functional limitations.


Intensive Rehabilitation Unit

The IRU has 27 specialized rehabilitation beds located at the HRSRH. Ten of the 27 beds are specifically designated for survivors of acquired brain injury (Slow to Recover and the Intensive Rehabilitation services).

Comprehensive rehabilitation management is provided by an interdisciplinary team of professionals for persons with some of the following conditions: stroke, amputation, acquired brain injury, neuromuscular disease, musculoskeletal disorders/arthritis/burns, chronic neurological conditions, spinal cord injury/disease, and multiple trauma-related conditions.


Outpatient Rehab & Support Services

The program offers a comprehensive and complementary variety of outpatient rehabilitation and support services including: physiotherapy and occupational therapy, driver assessment, prosthetics and orthotics, chiropody and nail care, seating and mobility clinic, adult augmentative communication clinic, chronic pain management program, hand and upper limb rehabilitation, outpatient brain injury services, Regional Evaluation Centre and Helpline Emergency Response Service.


Aquired Brain Injury (ABI)

The ABI service continuum consists of the following service options, following acute care stabilization:

  • two in-patient rehabilitation service streams (Slow to Recover and the Intensive Rehabilitation services), 
  • the Outpatient Brian Injury Rehabilitation Service, 
  • the Outreach Consultation Service

Service is provided by an interdisciplinary team of professionals. Each service is patient-centered and time-limited, utilizing a goal-directed approach towards functional gains. The Northeastern Ontario Regional ABI Coordinator works from the HRSRH, assisting with client access to all hospital ABI Rehabilitation Service components, as well as assisting in regional coordination of care. 


Assistive Communication Clinic

The Assistive Communication Clinic (ACC) is an accredited Assistive Devices Program (ADP) clinic (expanded level) for augmentative and alternative communication. The ACC provides assessment, consultation, intervention, and training to assist clients in developing functional communication (face-to-face and written communication).


Forward to Function

Forward to Function is an outpatient rehabilitation program for clients who most often have been involved in motor vehicle collisions or have suffered work-related injuries. We help clients progress towards their pre-injury level of functioning in their work, activities of daily living, mobility, leisure and social roles.

Forward to Function was formed in 2008, when a number of existing services within the Outpatient rehabilitation program were re-aligned. Each of the services within the Forward to Function array of services have a long standing history of excellence in meeting the needs of our clients and their families, physicians, the Workplace Safety and Insurance Board, and insurance companies.

Our multidisciplinary team of rehabilitation professionals is experienced in assessing and treating clients who have sustained complex neurological as well as musculoskeletal injuries.
